Millicent is a young woman afflicted by the Scarlet Rot who seeks answers about her origin. Her questline takes you across the entire Lands Between, from the rotting swamps of Caelid to the sacred grounds of Miquella's Haligtree. The quest culminates in a critical choice that determines which of two excellent talismans you receive.
Obtain the Unalloyed Gold Needle from Commander O'Neil at Swamp of Aeonia.
He repairs the needle. Give the repaired needle to Millicent at the Church of the Plague.
Millicent appears at the grace. Exhaust her dialogue.
Find the Valkyrie's Prosthesis on a corpse. Give it to Millicent at Erdtree-Gazing Hill.
Help Millicent defeat the Godskin Apostle boss. She is summonable for the fight.
Speak with her at the grace site.
FINAL CHOICE: Put down gold summon sign to help her (get Rotten Winged Sword Insignia) or red sign to invade her (get Millicent's Prosthesis).
If you kill the Ulcerated Tree Spirit at Haligtree before finishing the quest, Millicent dies. Gowry can be killed early — do not attack him. The choice at the end is irreversible, so pick based on your build.
Pro Tip
Help Millicent (gold sign) for Rotten Winged Sword Insignia — it is better for fast weapons like katanas, curved swords, and claws. Invade her (red sign) for Millicent's Prosthesis (+5 DEX) if you are doing a DEX build. You can only get one per playthrough.
Millicent's quest has a critical choice at the Haligtree. You can either help her by summoning to fight her sisters (reward: Rotten Winged Sword Insignia) or invade her after she removes the needle (reward: Millicent's Prosthesis + Unalloyed Gold Needle). The Rotten Winged Sword Insignia is better for most builds, providing up to 13% attack power increase on successive hits. Millicent's quest is the only way to obtain the Unalloyed Gold Needle, which transforms into Miquella's Needle if you inherit the Frenzied Flame. This needle can suppress the Frenzied Flame permanently when used in Dragonlord Placidusax's arena — a timeless space outside the influence of outer gods. This is the only way to reverse the Frenzied Flame ending lock and choose a different ending (like Age of Stars or Perfect Order). The needle is consumed on use, so choose wisely.
